3. Prepare for your Office Visit
In addition to completing the above forms, make sure you have the following for your visit:

• Insurance information
• Name and phone number of referring physician

4. Fill Out Your Medical History Form and Meet the Doctor!
Make sure to arrive at the office 10-15 minutes prior to the start of your appointment so that you can fill out your medical history form, that way we have a better understanding of your condition. Afterward, you'll meet with the doctor!
